Description

Vigamox Eye Drops is a sterile ophthalmic solution available in a 3 ml bottle. Each ml of the solution contains Moxifloxacin Hydrochloride 5.45 mg, equivalent to Moxifloxacin 5 mg (0.5% w/w base) as the active ingredient, along with boric acid, sodium chloride, and purified water as inactive components. The product is strictly for external ophthalmic use only and must not be injected or ingested. It is a broad-spectrum fluoroquinolone antibiotic used for the treatment of bacterial eye infections.

Moxifloxacin is a synthetic fluoroquinolone antibiotic that works by inhibiting two key bacterial enzymes, DNA gyrase (topoisomerase II) and topoisomerase IV. These enzymes are essential for bacterial DNA replication, transcription, and recombination. By blocking their activity, moxifloxacin prevents the bacteria from replicating their genetic material, effectively stopping bacterial growth and leading to cell death. This dual-target mechanism gives moxifloxacin a very broad spectrum of activity against both gram-positive and gram-negative bacteria, including common eye pathogens such as Staphylococcus aureus, Streptococcus pneumoniae, Haemophilus influenzae, Pseudomonas aeruginosa, and Moraxella catarrhalis. The drug also achieves good penetration into the cornea and aqueous humor, making it effective for both surface and deeper ocular infections.

The primary indication for Vigamox Eye Drops is the treatment of external bacterial infections of the eye, particularly bacterial conjunctivitis (commonly known as pink eye). It is also used for the treatment of bacterial keratitis (corneal ulcers), blepharitis, and other bacterial infections of the ocular surface. In clinical practice, it is also frequently prescribed as a prophylactic antibiotic before and after eye surgeries such as cataract extraction and LASIK to prevent post-operative infections. The product should not be used for viral or fungal eye infections, as it is ineffective against these pathogens. It is also not recommended for the prophylaxis or empiric treatment of gonococcal conjunctivitis due to the prevalence of fluoroquinolone-resistant Neisseria gonorrhoeae.

The recommended dosage for bacterial conjunctivitis is 1 to 2 drops instilled into the affected eye(s) three times daily for 7 days. For other bacterial eye infections, the dosage may be adjusted by the ophthalmologist based on the severity and type of infection. It is essential to complete the full course of treatment even if symptoms improve before the course is finished, to prevent the development of antibiotic-resistant bacteria. If a dose is missed, it should be instilled as soon as remembered, but if it is close to the time for the next dose, the missed dose should be skipped. The product should only be used under the supervision of a qualified ophthalmologist.

The most common side effects (reported in 1 to 6% of patients) include transient ocular discomfort or burning sensation upon instillation, conjunctivitis, decreased visual acuity, dry eye, keratitis, ocular hyperemia (redness), ocular pain, ocular pruritus (itching), subconjunctival hemorrhage, and tearing. These effects are generally mild and transient, resolving within a few minutes to a few days. Other possible side effects include blurred vision immediately after application, which usually clears within a few minutes, and an unusual taste in the mouth due to the solution draining through the nasolacrimal duct.

Serious but rare side effects include severe hypersensitivity reactions (rash, hives, swelling of face/lips/tongue, difficulty breathing), which have been reported with systemic fluoroquinolones and can occasionally occur even after the first dose of the eye drops. Prolonged or inappropriate use may result in overgrowth of non-susceptible organisms, including fungi. Patients should seek immediate medical attention if they experience severe eye pain, worsening redness, sudden vision changes, swelling around the eyes, or signs of a severe allergic reaction.

Important precautions include not wearing contact lenses during treatment, as the infection itself and the preservative in the drops can both damage the lenses. If using other eye medications simultaneously, a gap of at least 5 minutes should be maintained between different drops. Patients should avoid driving or operating machinery until their vision is clear after application, as temporary blurred vision is common. The dropper tip should not be touched to the eye or any surface to maintain sterility. The product should not be used if the solution changes color or becomes cloudy. The product is not recommended during pregnancy or breastfeeding unless the potential benefit clearly outweighs the risk, and consultation with a doctor is essential.

The product should be stored in a cool and dry place away from direct sunlight and kept out of reach of children. The bottle should be tightly closed after each use.
