Description
Xinep Eye Drops is a sterile ophthalmic suspension available in a 5 ml bottle. Each ml of the solution contains Nepafenac BP 0.1% w/v as the active ingredient, along with Benzalkonium Chloride Solution IP 0.02% w/v as a preservative and sterile aqueous buffered vehicle q.s. The product is strictly for external ophthalmic use only and must not be injected or ingested. The bottle should be shaken well before use. It is a topical non-steroidal anti-inflammatory drug (NSAID) prodrug used primarily for the management of post-surgical eye pain and inflammation.
Nepafenac is a non-steroidal anti-inflammatory prodrug that, after topical ocular dosing, penetrates the cornea and is rapidly converted by ocular tissue hydrolases to amfenac, its active metabolite. Amfenac is a potent NSAID that uniformly inhibits both cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes. These enzymes are responsible for the production of prostaglandins, which are natural chemicals in the body that mediate inflammation, pain, and swelling. By blocking prostaglandin H synthase (cyclooxygenase), amfenac effectively reduces the formation of prostaglandin precursors, resulting in decreased intraocular inflammation, pain, and redness. The prodrug design of nepafenac provides a novel drug delivery mechanism, allowing fast penetration through the cornea followed by rapid bioactivation to the active form at the site of action.
The primary indication for Xinep Eye Drops is the treatment of pain and inflammation associated with cataract surgery. It is used to prevent and treat the redness, pain, and swelling that commonly occur after this procedure. In diabetic patients, it is also used to reduce the risk of postoperative macular edema (swelling of a layer present at the back of the eye) associated with cataract surgery. The product is intended for use in adults and should only be used under the supervision of a qualified ophthalmologist.
The recommended dosage is one drop instilled into the conjunctival sac of the affected eye(s) three times daily (morning, midday, and evening), beginning 1 day prior to cataract surgery, continued on the day of surgery, and through the first 2 weeks of the postoperative period. For diabetic patients at risk of postoperative macular edema, the course may be extended up to 60 days post-surgery as directed by the ophthalmologist. The drops should be used at the same time each day for consistent effect. Patients should not use more drops than directed, and the course of treatment should be completed as prescribed.
The most common side effects include temporary stinging or burning sensation in the eye for 1 to 2 minutes upon instillation, eye redness, blurred vision, decreased visual acuity, foreign body sensation (feeling like something is in the eye), dry or sticky feeling in the eye, eye itching, and tearing. These effects are generally mild and transient. Other common side effects (1 to 5% incidence) include conjunctival edema, corneal edema, dry eye, lid margin crusting, ocular discomfort, ocular hyperemia, ocular pain, photophobia, and headache. Systemic effects are rare but may include nausea, vomiting, dizziness, and stuffy nose.
Serious but rare side effects include corneal erosion, corneal ulceration, corneal thinning or perforation (especially with prolonged use or in patients with pre-existing corneal problems), secondary cataract (clouding of the eye after surgery), increased intraocular pressure, and severe allergic reactions (rash, swelling of face/lips/tongue, difficulty breathing). Patients should seek immediate medical attention if they experience severe eye pain, worsening vision, persistent redness or swelling, or signs of a severe allergic reaction. Using the drops more than 1 day before surgery or for more than 14 days after surgery (unless directed for diabetic macular edema prevention) could increase the risk or severity of corneal side effects.
Important precautions include removing contact lenses before instilling the drops and waiting at least 15 minutes before reinserting them, as the preservative may be absorbed by soft contact lenses and cause discoloration. If using other eye medications simultaneously, a gap of at least 5 to 10 minutes should be maintained between different drops. Patients should avoid driving or operating heavy machinery until their vision is clear after application, as temporary blurred vision is common. The product is contraindicated in patients with a previously demonstrated hypersensitivity to any of the ingredients in the formula. It should be used with caution in patients with diabetes, arthritis, bleeding disorders, or dry eye syndrome. The product may increase bleeding time due to interference with platelet aggregation. The solution should not be used if it changes color or becomes cloudy. The product is not recommended during pregnancy or breastfeeding unless the potential benefit clearly outweighs the risk.
The product should be stored in a cool, dark place away from direct sunlight and kept out of reach of children. The bottle should be tightly closed after each use. The product is available in India at a price range of approximately 130 to 200 rupees per 5 ml bottle and is available through major online pharmacy platforms and offline pharmacies with a valid prescription.
