Dry eye is a multifactorial disease and is accompanied by increased osmolarity of the tear film and inflammation of ocular surface.

Tear hyperosmolarity results from reduced aqueous tear flow (aqueous-deficient dry eye) and/or increased evaporation of the aqueous tear phase from the exposed ocular surface (evaporative dry eye). Tear hyperosmolarity is a principal step in the vicious circle of dry eye disease (DED) pathology. Tear hyperosmolarity leads to morphological changes such as apoptosis of cells of both the conjunctiva and cornea. It is also associated with inflammatory events, which lead to further cell death and loss of mucin secreting goblet cells

The symptoms of dry eye vary among patients, and most commonly they include itching, grittiness, burning, and sensitivity to bright light, foreign-body sensation, irritation, pain, blurred vision, and contact lens intolerance

The symptoms of Dry eye can be managed and complications can be prevented by the use of artificial tears and lubricating gels that restore normal tear film. In general, artificial tears moisturize the eye surface by simply increasing the water content of the tear film or by preventing tear evaporation.

For the episodic or transient dry eye, artificial tears are often used to alleviate symptoms of dry eye, but are palliative. Osmoprotection is a new approach to treating dry eye by providing protection against hypertonicity below the corneal surface via hydrating the epithelial surface.

To compensate for hypertonic conditions, several compatible solutes have been incorporated into topical formulations for the treatment and management of DED. These are organic compounds that work like electrolytes to balance osmotic pressure, yet do not interfere with cellular metabolism, thus aiding survival of organisms under extreme osmotic stress. E.g.: L-carnitine, erythritol, glycerine.

Osmodrops contains Carboxymethyl cellulose 0.5% and glycerine 0.9% as active ingredients and L-carnitine and erythritol as excipients with Stabilized Oxychloro Complex (SOC) which is a safe preservative. It is indicated for dry eyes. The recommended dosage is 1-2 drops in the affected or as needed by the patient. Contraindications include hypersensitivity to sodium carboxymethyl cellulose or to any of the excipients.
