Description
Pred Forte Eye Drops is a sterile ophthalmic suspension available in a 10 ml bottle. Each ml of the suspension contains Prednisolone Acetate USP 10 mg (1% w/v) as the active ingredient, along with Carbomer 940 0.01 g, Phenylephrine Hydrochloride 0.006 g, Phenylmercuric Nitrate 0.001 g, Sodium Chloride 0.006 g, and Sodium Hydroxide and/or Hydrochloric Acid to adjust pH as inactive components. The product is strictly for external ophthalmic use only and must not be injected or ingested. The bottle should be shaken well before each use. It is a potent topical corticosteroid (glucocorticoid) anti-inflammatory agent used for the management of severe inflammatory conditions of the eye. The product is packaged in a white and red box with the brand name “PRED FORTE” displayed prominently in bold black text, with the product description “Prednisolone Acetate Ophthalmic Suspension USP” and a “STERILE 10 mL” indication.
Prednisolone Acetate is a synthetic glucocorticoid corticosteroid that is the active metabolite of prednisone. Its mechanism of action is based on its ability to bind to intracellular glucocorticoid receptors in target cells. Once bound, the drug-receptor complex translocates to the nucleus and modulates gene transcription, leading to the inhibition of the release of inflammatory mediators such as prostaglandins, leukotrienes, and cytokines. It also suppresses the migration of polymorphonuclear leukocytes and fibroblasts to the site of inflammation, reverses capillary permeability, and stabilizes lysosomal membranes at the cellular level. This multi-targeted action results in potent anti-inflammatory, immunosuppressive, and anti-allergic effects in the eye. The acetate ester form provides a microfine suspension with prolonged residence time on the ocular surface, ensuring sustained therapeutic action. The small amount of phenylephrine hydrochloride in the formulation provides mild vasoconstriction, helping to reduce conjunctival redness.
The primary indications for Pred Forte Eye Drops are the treatment of severe inflammatory conditions of the eye, including severe allergic conjunctivitis, severe non-infectious keratoconjunctivitis, severe uveitis (inflammation of the uveal tract), and severe post-surgical inflammation following ophthalmic procedures such as cataract extraction. It is also used for the management of severe post-traumatic inflammation, chemical burns of the eye, and other severe inflammatory eye conditions that have not responded adequately to milder corticosteroid preparations. The product is intended for use in adults and is not approved for use in children below 18 years of age. It is not intended for long-term use and should only be used under the supervision of a qualified ophthalmologist.
The recommended dosage is 1 to 2 drops instilled into the conjunctival sac of the affected eye(s) 2 to 4 times daily, or as directed by the ophthalmologist. During the initial 24 to 48 hours of treatment for severe inflammation, the dosage may be safely increased to 2 drops every hour. The dosage and length of treatment are based on the patient’s medical condition and response to treatment. Once the inflammation is controlled, the frequency should be gradually reduced to the minimum effective dose. Eye drops containing corticosteroids should not be used for more than 10 days except under strict ophthalmic supervision with regular checks for intraocular pressure and other complications. Patients should not stop the medication abruptly after prolonged use without consulting a doctor.
The most common side effects include transient burning or stinging upon instillation lasting 1 to 2 minutes, temporary blurred vision, eye irritation, foreign body sensation (feeling like something is in the eye), eye redness, and a change in taste (dysgeusia). Other common side effects include headache, eye itching, and mild eye pain. These effects are generally mild and transient. Uncommon side effects may include increased intraocular pressure, conjunctivitis, keratitis, corneal ulceration, and visual disturbances.
Serious but less common side effects include elevation of intraocular pressure with possible development of glaucoma and infrequent optic nerve damage, posterior subcapsular cataract formation, delayed wound healing, secondary ocular infections (bacterial, fungal, and viral), and perforation of the globe when used in conditions where there is thinning of the cornea or sclera. The development of secondary ocular infection is a significant risk, particularly with prolonged use. Patients should seek immediate medical attention if they experience severe eye pain, sudden vision loss, blurred vision, tunnel vision, seeing halos around lights, or signs of a new or worsening eye infection.
Important precautions include not wearing contact lenses during treatment, as the preservative may be absorbed by soft contact lenses. If using other eye medications simultaneously, a gap of at least 5 to 10 minutes should be maintained between different drops. The product is contraindicated in acute untreated purulent ocular infections, in most viral diseases of the cornea and conjunctiva including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella, and also in mycobacterial infection of the eye and fungal diseases of ocular structures. It is also contraindicated in individuals with known or suspected hypersensitivity to any of the ingredients or to other corticosteroids. Intraocular pressure should be checked frequently during treatment, especially if used for 10 days or longer. The product may cause temporary blurred vision, so patients should avoid driving or operating machinery until their vision is clear. The product is not recommended during pregnancy or breastfeeding unless the potential benefit clearly outweighs the risk. The suspension contains sodium bisulfite, a sulfite that may cause allergic-type reactions including anaphylactic symptoms in susceptible individuals, particularly those with asthma.
The product should be stored in a cool and dry place away from direct sunlight and kept out of reach of children. The bottle should be tightly closed after each use. The product is available in India at a price range of approximately 150 to 250 rupees per 10 ml bottle and is available through major online pharmacy platforms and offline pharmacies with a valid prescription.
